Prior to your appointment:
• Please verify that we accept your insurance plan.
• Please verify that your plan is current.
• If your insurance plan requires a referral from your primary provider, please verify that we have received that referral.

For each appointment:
• Bring your current insurance card.
• Co-payments and deductibles are due
on same day that the service is provided.
• Bring your current driver's license or photo ID.

In addition to your insurance, we accept cash, checks, visa, discover and mastercard.

Fees:
Fees are dependent upon each service provided. Laboratory, x-ray, and pathology services are billed separately. If you have questions about the costs involved, please contact our patient account representative at 413-8250.
