How Do I
Schedule An Office Appointment?
Our main office is located in Olympia, with
a satellite office in Shelton. Appointments can be made directly with our Patient Care
Coordinator by calling the office closest to you.
What
To Bring To An Appointment:

Bring a list of all medications you currently take, including over the
counter and herbal supplements. Include the exact name of medication, dose
of medication, and how often you take it. If it is more convenient, you may
bring your medication in its original container.
If you have any pertinent lab or x-ray reports that will assist in your
evaluation, please ask your primary care provider to send those to us prior
to your appointment.
Bring your current insurance card. Co-payments and deductibles are due on
same day that the service is provided.
Prescription
Refills:

In order to expedite your request to refill a prescription prescribed by one
of our providers, call your pharmacy and ask them to Fax us a request. Allow
2-3 business days to complete this process, as we need to review your chart.
We may ask you to come and see your provider. Patients receiving medication
long term must be seen yearly. If you were provided samples, leave us the
name of the medication you were given along with the name and location of
the pharmacy you want your prescription called in to.

Obtaining
Test Results:

We will notify you of your test results (lab and/or x-ray) by telephone,
mail, or at your next scheduled appointment. Please call our office if you
have not received your results within 2 to 3 weeks.

Obtaining
Procedure Results:
You will be given written discharge instructions that include what was found
during your procedure, before you leave the endoscopy center. Pathology
results will be mailed to you within 14 business days. Your referring
provider will also receive a copy. Please call our office if you have not
received your results within 3 weeks.
